I certify that I will ensure that my Patriot Pet is spayed or neutered at my expense within three months of entry into the United States, should the animal not have been already sterilized in SPCA International’s care. *

I understand

I certify that I will be responsible for all costs related to the care and wellbeing of my Patriot Pet once delivered to me, including but not limited to food, supplies, veterinary care, vaccinations, and any necessary training. *

I understand

I certify that my Patriot Pet will be treated humanely and will be provided with a loving home; that I will ensure that my Patriot Pet is provided proper food, water, shelter, veterinary care, exercise, love, attention, and kind treatment at all times; that my Patriot Pet will not be sold or used for any experimental, commercial or inhumane purpose; ;will not be subjected to any cosmetic surgery, (e.g.earcropping and tail docking) declawing or debarking and will not be used for dog fighting or any similar activity. *

I certify that if I can no longer keep or care for my Patriot Pet, I will contact SPCA International immediately and that I will not sell or give this pet to any other person, friend, relative, organization, or animal shelter. *

I understand that SPCA International will take every reasonable precaution to ensure the health of my Patriot Pet from intake into the program until final delivery to my home, and I will not hold SPCA International liable for any unforeseen health complications, illnesses, injuries, or death that may result while my animal is in the care of the organization or its partners. *

I understand

I, the designated caregiver/owner of the above described animal, give permission to SPCA International and their representatives to transport my animal from originating country to the United States as part of the Patriot Pets: Worldwide program. I understand that SPCA International has taken, and will continue to take every precaution possible to safe guard my animal between the time they take possession of my animal until their final destination in the United States. However, I understand that SPCA International cannot make any guarantees that my animal will not get sick, injured or even die while in the care of the organization’s representatives. If something were to happen to my animal while in the care of SPCA International, I give the organization’s representatives permission to get necessary veterinary care. If I cannot be reached, the extent of the veterinary care that my animal receives is a decision that I give to SPCA International and its representatives in conjunction with the recommendation of the veterinarian providing treatment. If everything medically possible has been done for my animal and he/she is suffering, I give SPCA International and its representative permission to have a veterinarian humanely euthanize my animal if they are unable to reach me and if prolonging this decision would cause my animal further suffering.
